No endorsement is made. Listing a provider doesn't mean we recommend it. Before you begin, review the NPI record and the provider's credentials, and check its license.
Accuracy. Public sources supply our records, and those records may be out of date. When we're told about a mistake, we fix it, but we can't promise that all details are current.
Using the list fairly. For your own family's search needs, you may use the site. Bulk scraping and republishing aren't allowed.
Links. Our links lead to state agencies, insurers and providers. We have no control over their sites.
Changes. We may change these terms, and the version on this page is the one that applies.
